How can I add an approve or reject button to my email notifications?

In a recent webinar a customer asked how they might use an email notification to embed an “approve” and “reject” button such that when their client opened their email, they could make the selection right there in the email and the result would be to update the record and log the client’s choice to include an audit trail of the person, the decision date and time.

Example with an Approve and Reject button.

You can do this with a simple Formula URL button.

    I believe I understand where your error might come from. I just got a new copy of the Customer Projects and Documents application Kirk mentions in his video and duplicated the functionality. Since you're not getting the app token error I assume you've added that to your app token list properly since that was the first change I had to make to the default application. The second major change, after adding the two URL formula buttons and the Approval Status field, was to check the field IDs. The two URL fields in my version were field 98 and 99 and my Approval Status field was field 100. Kirk's video has the code affecting field 97 which is why I think your record is not being updated.
